Cascade3d Connected Care Reablement Case Study with Warwickshire County Council

Project Overview

Warwickshire residents keeping independent

A new system of smart devices is being used by the Reablement Team from Warwickshire County Council to support wellbeing and outcomes for local people. ‘TEC (Technology Enabled Care) Me Home’ has been in use since March 2025 by the Warwickshire Reablement Service who are using this new technology alongside their normal care and support for people recently discharged from hospital or recovering from a condition that’s affected their independence.

The TEC Me Home system uses discreet motion sensors, temporarily installed in a person’s home with their full permission and agreement, to monitor movement throughout the property. This data provides the Reablement Team with crucial insights into daily routines, including sleep patterns, personal care activities in the bathroom, and food and drink preparation in the kitchen.

As well as the team having access to the movement information, the person’s family can also receive updates from a mobile phone app.

The system is provided by a company called Cascade3d who work across the UK with county councils providing smart technology to keep people living independently at home for longer.

The project in Warwickshire has seen some great results with many people benefitting from the Reablement service’s use of this technology. In one example a person’s family had raised concerns about their mother not eating. They had identified that food which had been brought had not been eaten, and they had concerns about the person’s nutritional intake and increased forgetfulness.

The sensors were installed as part of the Reablement offer. After a week of monitoring the assessor and family gained clear evidence that the person was actively participating in initiating food preparation tasks, identified by sensors in the fridge, on the freezer door and smart plugs on the microwave and kettle. It was agreed by the person, family and assessor that no care package was required at this time due to the evidence provided by the sensors. The sensors were left in the person’s home for a further two weeks to provide reassurance to the family before being removed and used again in another home.

Claire Lydon, Occupational Therapy Team Manager, commented “The team have been enthusiastic to use this new technology with the people accessing the Reablement Service within Warwickshire. It has allowed us to have evidence of a person progressing their independence during their Reablement journey as well as identifying people who may need some extra support. We are excited to continue using TEC Me Home to support more people across Warwickshire and combine care with technology to make our service the best it can be.”

Case Study

Henry’s Story

Henry was referred to Reablement after a fall that affected his confidence and led to changes in his daily routine. Henry’s family were concerned about the stairs at home and arranged for him to live downstairs. His daughter informed the Reablement Officer that she was very anxious about leaving her father alone and wanted the maximum of four daily Reablement calls.

The Tec Me Home sensors were discussed as an option and both Henry and his daughter consented to the kit being installed. The daughter installed the app on her phone so that they both could monitor the system in real time. The Reablement Officer observed an immediate sense of relief from the daughter, who was able to check in with her father at any time, providing peace of mind. Henry informed the Reablement Officer that he felt confident returning to his routine, knowing his daughter could monitor him remotely, promoting his independence.

The Reablement Officer was able to gather evidence from the sensors, which gave them confidence to end the Reablement calls, leaving Henry fully reabled and both him and his daughter satisfied. It was agreed that the sensors would remain in situ for an extended period, enabling the daughter to go on holiday while still checking in on her father from the comfort of her sun lounger!

Case Study

Kevin’s Story

Kevin started his reablement journey following a discharge from hospital after suffering with gallbladder pain and infection around his drain site in his abdomen. Kevin lives alone and has 3 sons that live fairly local. During the initial assessment the son was present and voiced family were worried about Kevin being alone within the property. The Reablement officer discussed the Tec Me Home equipment and the benefits that it has and how it gives family peace of mind. Kevin and son were very accepting of the equipment and gave their consent for installation.

Sensors were installed in the lounge, bedroom, bathroom, kitchen, on the kettle, microwave and in fridge. A door sensor was installed to the back door, this was very important to the family as Kevin likes to tend to his garden and one of their main worries was that he would exit the back door and fall. Kevin does not have a lifeline so the sensor would pick up any motion going out and motion coming in. The son installed the app on his phone and advised his brothers to do the same.

The son is owner and company director of a fire prevention company, and he was very impressed with how he was able to monitor his dad's movements throughout the day and night. We have been able to complete the Kevin’s reablement journey early as he is now independent with goals established at the beginning of his reablement and the Tec Me Home kit has been a vital support for the family in that they have felt happy for Reablement to finish calls as they are still able to have the kit monitoring until the end of the 6 weeks. The son has also expressed interest in purchasing a kit.
